All documents can be filled out online and brought to registration or mailed to us at P.O. Box 95, Hamlin, NY 14464 .
Documents are also PDF fillable online to save you time writing them out.
Copies will be available at registration.

NOTE: YOU WILL BE REQUIRED TO PROVIDE YOUR OWN SCHOOL APPROVED PHYSICAL FORM STATING THAT YOUR CHILD IS ABLE TO PARTICIPATE!

Business Sponsorship Form  (form for those businesses who would like to sponsor us and have an ad placed in the yearbook.)
